After you enter the data (please put Check # and Date in the “Notes” field) into this form, open (click on) the “Not Used” button on lower left and enter into the dropdown box after Sponsor, -Check. On this entry the “Click Here To Make Payment” tab goes away. You can now hit the “CLICK TO SUBMIT” Button to enter your check sponsor entry.
After Submission you should get a Submission successful message. Disregard the message that your credit card will be charged. You are now done with the entry.
You should get an email confirm of your entry. After Submitting the Sponsor Data, please mark the hand-in order entry form as submitted and give check and submitted form to Shelly.
