This form will enter the sponsor raffle ticket. No charge will be shown and no credit card option will be offered.  Place the payment mehtod in the Notes section of the Sponsor Raffle Order.

The Sponsor Level tics as follows: $10,000 – 10 Tics    $5,000 – 5 Tics   |   $2,500 – 4 Tics  |  $1,000 – 3 Tics    $500 – 2 tics  |  $250 – 1 tic
